National Data Set (NDS) for 2006

NDS 2007
The NDS 2007 data submission worksheet will be posted on the NDS page by February 1, 2008. Online data submission will begin March 1, 2008 and end April 15, 2008. Visit the National Data Set page of the NHPCO website periodically for ongoing NDS updates. Every hospice's data enhance the value of the NDS; therefore, all hospices in Iowa (including non NHPCO members) are urged to join in the submission process. The more information hospices submit, the better the NDS can meet the hospice community’s needs.
NHPCO maintains a dedicated email address (nds@nhpco.org) for questions related to the survey, data submission, or any other part of NDS process should you need individual assistance.

A one-page benchmark report that includes a comprehensive set of performance
measures including quality outcomes, patient volumes and mix, quality operations
and quality practices. QAPI Snapshot tracks agency-wide performance, as required
by the new CoP and supports major national hospice quality efforts, including
the NHPCO Quality Partners initiative. For each report, you simply complete
an on-line survey and, after a short processing time, download the completed
benchmark report. QAPI Snapshot surveys will capture historical data to provide
you with a baseline and trends in your first report.
