HOSPICE CARE CONSULTANT FOR CEDAR RAPIDS

The Hospice Care Consultant is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Hospice Compassus philosophy, using the 6 Pillars of success as the foundation.  Under the direction of the Executive Director or Director of Sales, the Hospice Care Sales Consultant is responsible for development of the hospice program through direct community contacts for the purpose of educating healthcare providers as well as the general public about the hospice program.

Responsibilities Include:

·        Provides accurate information regarding hospice services in response to inquiries by healthcare providers and general public.

·        In conjunction with the Executive Director, Director of Sales (if applicable) and the Regional Director of Sales, prepares a goal directed development plan for the purpose of educating health care providers and the general public about the hospice program.

·        Promotes positive public relations with patients, family members and guests.

·        Maintains current data on market area, competitors, and marketing strategies.

·        Maintains an organized approach to territory management.

·        Prepares and conducts calls and presentations to potential referral sources.

·        Participates in strategic planning and the analysis for their assigned territory in conjunction with the hospice agency business plan.

·        Coordinates with clinical management staff in planning in-services, presentations, and in addressing issues with referral sources.

·        Initiates and coordinates contract negotiations with facilities, insurance companies, and managed care organizations.

·        Participates in community and organizational programs as requested to promote professional growth and understanding of hospice care.

·        Promotes an atmosphere which allows for the privacy, dignity and well-being of all patients in a safe, secure environment. 

Requirements:

·        Experience in health care sales and\or marketing preferred.  One to two years experience and/or training; or equivalent combination of education and experience. 

·        Demonstrated proficiency in skills related to public relations, sales, and marketing.  Ability to work independently in the management of assigned responsibilities.  Possess a valid driver's license and auto insurance

·        Travel is required, but no overnight travel.

·        Must be able to travel frequently; however, no overnight travel required.

EXECUTIVE DIRECTOR FOR CEDAR RAPIDS

 

The Executive Director is responsible for implementing corporate policy and mission at the Hospice program level by maintaining appropriate staffing levels, meeting set corporate standards and goals and assuring the long term fiscal viability of the program.

 

Responsibilities Include

 

·        Function as the senior executive for the Hospice program.

o   Managing hospice personnel in a manner that minimizes stress, promotes quality service, and provides a professional work atmosphere, with minimum staff turnover.

o   Implementing the financial/budget guidelines set by corporate.

o   Controlling variable expenses so that program expenses do not exceed revenues, thus assuring a long term and viable hospice program.

o   Organizes and facilitates regular management meetings with the goal of positive, motivating team building, as well as management problem solving and peer support.

o   Demonstrates support of the corporate mission (by actions and attitude), policies and procedures and assures consistent compliance within the hospice program.

 

·        Provides regular and appropriate documentation/communication to corporate office.  

o   Provides accurate and timely billing and vendor service information

o   Provides accurate and timely patient services related information.

o   Prepares end of the month status reports for the Regional Director of Operations/Divisional Vice President as well as timely, detailed and accurate marketing plans, logs and supporting documentation files.

 

·        Ensures compliance with all local, state and federal Hospice and Hospice care related regulations and mandates.  

o   Facilitates all appropriate regulatory and internal audits and reviews and provides appropriate responses/documentation to any and all appropriate regulatory authorities and or corporate departments to assure accurate and timely compliance and or corrective action, thus assuring legal, ethical, quality and consistent patient care.

o   Negotiates implements and monitors all services provided locally through contractual agreements, while providing the corporate office with copies of all such current and executed service/contractual agreements.

 

·        Administer agency personnel program.    

o   Coordinates employee recruitment, hiring, appraisals, discipline and discharges with and through corporate Human Resources Department to assure legality and consistency with personnel policies and procedures.

o   Assures the appropriate administration of employee disciplinary action as dictated by policy and procedure, with appropriate technical guidance from the corporate Human Resources Department.

·        Promotes program growth and development.   

 

Requirements Include:

·      A minimum of a Bachelor's Degree in either Health Care Administration, Business Administration or in a related Human Services field. A Masters Degree in one of these fields is preferred.

·      A minimum of five years senior management experience in home health or hospice. Seven years experience would be preferable.

·      A working knowledge of Federal and State regulations as they pertain to the provision of comprehensive, hospice home care, its administration, and related reimbursement.

·      Understanding and acceptance of hospice philosophy.

·      Working knowledge of EEOC regulations, human resource management, benefits management and risk management, as they pertain to health care employees.

·      Possess strong leadership, organizational and interpersonal skills.

Care Coordinator, RN for Iowa City Hospice

 

 

Iowa City Hospice has an opening for a full-time Care Coordinator, RN, who is responsible for the day-to-day coordination of patient and family care on a daily basis through serving as the in-house communication source for the patient, family and Iowa City Hospice staff. Iowa City Hospice was established in 1983 and is a community-supported nonprofit organization serving a seven county area.

 

Responsibilities include:

1.      Coordinate phone calls from the patient, family and staff to ensure adequate assessment, planning and implementation of each patient’s and family’s plan of care.

2.      Coordinate the patient’s plan of care from the referral, admission through coordinating inpatient care, discharge and transfers. 

3.      Work with Care Partners to ensure transition from settings and levels of care are smooth.

4.      Order equipment and transportation to meet the needs of the patient/family as approved in the plan of care. 

5.      Schedule the interdisciplinary team to ensure coordinated care and efficient use of resources.

6.      Ensure compliance with Standards of Care and the Medicare Conditions of Participation.

7.      Schedules daily nurse visit coverage; works with the nursing supervisor to arrange on-call and PRN nurse coverage for patient visits.

 

Schedule: Monday through Friday, 8 a.m. – 5 p.m. plus Administrative on-call duties

 

Qualifications: 

  1. Current Iowa License as a Registered Nurse in Iowa.
  2. BSN degree or Associate degree with relevant experience
  3. Minimum of two years of nursing experience, hospice or home care experience preferred.
